Moved Recently?

When your address changes, submit a Change of Address Form to the Members’ Records Department at the Joint Industry Board. This change also applies to the Deferred Salary Plan.  The Joint Industry Board will notify Empower of this change of address. Please be sure to keep your contact information current with the Joint Industry Board at all times.

Birth of a Child?

To add a newborn child to your Medical and Dental Plans, submit an Enrollment Form and a copy of the birth certificate to the Members’ Records Department at the Joint Industry Board within 30 days of the birth of the child. In addition, you may want to consider updating your beneficiary information.

Adoption of a Child?

To add a newly-adopted child to your Medical and Dental Plans, submit an Enrollment Form to the Joint Industry Board upon the child’s adoption or placement for adoption. In addition, you may want to consider updating your beneficiary information.

Child’s 19th/26th Birthday?

* The Dental Benefit Plan of the Electrical Industry and the Dental Benefit Plan of the Elevator Industry (“The Dental Plans”): Under the Dental Plans, coverage for an unmarried dependent child who is a full-time student ends when the child reaches age 26.

For COBRA continuation of your child’s dental coverage, notify the Members’ Records Department at the Joint Industry Board within 60 days after the child’s eligibility ends. Your child will then be required to submit a COBRA Application Form to the Joint Industry Board within 60 days after your child is notified of his/her right to elect COBRA continuation coverage.

Child’s 26th Birthday?

Coverage for a dependent child ends when the child reaches age 26.* For COBRA continuation of your child’s medical coverage, you must notify the Members’ Records Department at the Joint Board within 60 days after the child’s eligibility ends. Your child will then be required to submit a COBRA Application Form within 60 days after the Joint Board notifies your child of his/her right to elect COBRA continuation coverage.

*This rule is effective 10-1-10 for PHBP participants and 1-11-11 for ESF participants.

Child Left School?

Under the Dental Benefit Plan of the Electrical Industry and the Dental Benefit Plan of the Elevator Industry, coverage for an unmarried dependent child over age 19 ends on the day the child is no longer a full-time student. For COBRA continuation of your child’s dental coverage, notify the Members’ Records Department at the Joint Board within 60 days after the child’s eligibility ends. Your child will then be required to submit a COBRA Application Form to the Joint Industry Board within 60 days after your child is notified of his/her right to elect COBRA continuation coverage.

Child Graduated?

Under the Dental Benefit Plan of the Electrical Industry and the Dental Benefit Fund of the Elevator Industry, coverage for an unmarried dependent child over age 19 ends on the day the child is no longer a full-time student. For COBRA continuation of your child’s dental coverage, notify the Members’ Records Department at the Joint Board within 60 days after the child’s eligibility ends. Your child will then be required to submit a COBRA Application Form to the Joint Industry Board within 60 days after your child is notified of his/her right to elect COBRA continuation coverage.

Child’s Marriage?

Under the Dental Benefit Plan of the Electrical Industry and the Dental Benefit Plan of the Elevator Industry, coverage for a dependent child ends on the day the child marries. For COBRA continuation of your child’s dental coverage, notify the Members’ Records Department at the Joint Board within 60 days after the child’s eligibility ends. Your child will then be required to submit a COBRA Application Form to the Joint Industry Board within 60 days after your child is notified of his/her right to elect COBRA continuation coverage.

Marriage?

If you marry, submit an Enrollment Form and marriage certificate to the Members’ Records Department at the Joint Industry Board within 30 days of your marriage. In addition, you may need to update your beneficiary information. for your benefit plans.

Divorce?

If you get divorced, submit an Enrollment Form and a copy of your final divorce decree to the Members’ Records Department at the Joint Industry Board as soon as possible after your divorce is effective. You also may want to contact the Joint Industry Board to discuss how Qualified Domestic Relations Orders (QDROs) and Qualified Medical Child Support Orders (QMCSOs) affect your benefits. In addition, you may need to update your beneficiary information. For COBRA continuation of your ex-spouse’s medical and dental coverage, the Joint Industry Board must be notified within 60 days after the divorce. Your spouse will then be required to submit a COBRA Application Form to the Joint Industry Board within 60 days after he/she receives notification of his/her right to elect COBRA continuation coverage.

Death?

In the event of a participant’s death, the participant’s spouse or other beneficiary must notify the Joint Industry Board and submit a certified copy of the participant’s death certificate as soon as possible. In the event of a dependent’s death, you should inform the Members’ Records Department and update your beneficiary information, if necessary.

Change of Phone Number?

When your phone number changes, notify the Members’ Records Department at the Joint Industry Board. To change your phone number for the 401(k) Plan, call Empower at (877) JIB-401K or (877) 542-4015. Please be sure to keep your contact information current at all times.

Left Covered Employment?

For COBRA continuation of medical and dental coverage, submit a COBRA Application form to the Members’ Records Department at the Joint Industry Board within 60 days of your notification of your right to elect COBRA continuation coverage.

Become Disabled?

If your Workers’ Compensation and/or disability benefits are provided by E.E.S.I.S.P., you must contact them immediately at (718) 591-2000, Extension 1250.

To apply for Supplementary Disability Benefits from the Additional Security Benefits Plan, submit an Additional Security Benefit Plan Enrollment Form or contact the Joint Industry Board as soon as you are unable to work due to a non-work-related illness or injury. If you become totally and permanently disabled, you may elect a distribution from your 401(k) and/or Annuity Plan or elect to receive benefits from the Pension Plan, if you are eligible.

Retirement?

If you’re thinking of retiring, contact the Pension Department at the Joint Industry Board at (718) 591-2000, Extension 1280, to request a pension estimate and a pension application to get the process started. When you’re ready to retire, you must contact the Joint Industry Board at least 60 but not more than 90 days before you want pension payments to begin (unless you are applying for a disability pension). To claim benefits from the Annuity Plan, the Additional Security Benefits Plan, the Health Reimbursement Account Plan and the Vacation, Holiday and Unemployment Plan, contact the Joint Industry Board at (718) 591-2000, Extension 2222. Contact Empower directly at (877) 542-4015, regarding distributions from your 401(k) Plan.

Becoming Eligible for Medicare?

If you are an active member of the PHBP Medical Plan and over age 65, you will continue to be covered by the Plan as you were before age 65. If you are a retired member of the Plan, this Plan will act as Secondary Payor to Medicare and will include some coordinated benefits. If you are not eligible for health coverage upon retirement, the Joint Industry Board will notify you of your right to elect COBRA continuation coverage. You will then have 60 days from that notice to complete a COBRA Application Form.
